CFOH Forms
These forms must be completed and reviewed prior to acceptance into Children’s Flight of Hope. In additional, these forms will be updated annually by all returning families.
Air Transportation Approval Form- REQUIRED
Children’s Flight of Hope verifies that each child is medically cleared to fly on a commercial plane before providing flight assistance.
This form must be completed by a medical doctor, physician assistant, or nurse practitioner.
Financial Verification Form or Government Subsidy- REQUIRED
Children’s Flight of Hope ensures that we assist families experiencing financial hardship, which would otherwise limit their ability to pay for flights to medical care.
This form must be completed by a social worker/case manager, your child’s referring physician’s office, or other relevant professional.
OR if you or your child receives any form of government subsidy (Medicaid, SNAP, SSI, etc.), please email a copy of the award letter or card to missions@cfoh.org
Family Financial Questionnaire- REQUIRED
This is a new questionnaire used for our organization to gather data and information so that we can better serve our families
Urgent Medical Appointment Form- REQUIRED ONLY IF TRAVEL IS LESS THAN 14 DAYS AWAY
1. CFOH requires that flight requests be submitted at least 14 days prior to departure. This requirement is for EVERY flight request of both new and returning clients. If circumstances require less than 14-day notice, this form is required. Completing this form does not automatically approve this request for exception.
*This policy does not apply to a rebooking of a flight due to airline cancellations or extended stays in the hospital.
2. This form will only be accepted if signed by the child’s medical doctor, case manager, social worker, or another relevant professional on the child’s medical team. Requests without proper certification will NOT be processed.
